When one is praying (the Shemoneh Esrei), he should not hold in his hand tefillin, nor a book of the Holy Scriptures, nor a full bowl, nor a knife, coins, or a loaf of bread, because his heart is upon them, worried lest they fall, so that he would become distracted and his concentration (kavanah) would be lost. But a lulav, in its season, he is permitted to hold in his hand, since holding it in his hand is itself a mitzvah, he is not distracted on its account.
One is permitted to hold a prayer machzor in his hand while he prays, since he is holding it for the sake of the prayer itself, he is not distracted, similar to the lulav, that because taking it is a mitzvah he is not distracted. The Rema adds: Nevertheless, if it was not already in his hand before he began, he should not turn back for it during the prayer to take it, unless it was in a designated place where it is ready at hand, in which case he is permitted to take it even in the middle of the prayer, in order to pray from it (the הר"י, in the chapter Mi Shemeto).
(1) 'When one is praying' — the Pri Megadim writes that the same law applies during the recitation of the Shema and Pesukei DeZimra.
(2) 'He should not hold' etc. — after the fact he need not go back and pray again, unless he knows that on account of this he did not concentrate in (the blessing of) Avos. The chazzan (shaliach tzibbur) is permitted to hold the sefer Torah in his hand on Shabbos when he says Yekum Purkan, for since his intention then is to pray on behalf of those who study Torah, he is therefore holding the sefer Torah in his hand, and not for the sake of guarding it, and this is like a lulav in its season. Likewise he is permitted to hold it at the time they bless the new month (Birkas HaChodesh).
(3) 'Nor a book' etc. — and likewise all of our (printed) books.
(4) 'A full bowl' — his heart is upon it, worried lest its contents spill from it; and with a knife, lest it fall to the ground and pierce his foot; and with coins, lest they scatter; and even if they are bound up (in a bundle) he is distracted lest they be lost, unless he holds them in a manner in which there is no concern that they will be lost. The ברכ"י writes that the same applies, that it is forbidden to seat a small child before him at the time of prayer.
(5) 'And coins and a loaf' etc. — specifically these, which if they fall involve a monetary loss or would injure him, but other objects he is permitted to hold. And some say that these are not meant to be exclusive, but rather the Shulchan Aruch mentioned what is common, and likewise one should not take any object at all in his hand at the time of prayer; and so the Taz agreed.
(6) 'Because his heart' etc. — it requires further examination (tzarich iyun) whether one is permitted to take tefillin or a sefer Torah in his hand in a case where he is afraid of thieves, for perhaps they permitted it to him [the Pri Megadim].
(7) 'One is permitted to hold' etc. — if a book fell to the ground and he cannot concentrate, he is permitted to pick it up when he finishes the blessing in which he is currently standing, but if that is not the case, he should not interrupt; and all the more so that one should not signal with a finger or the like during the prayer. If he began to pray the Shemoneh Esrei and became confused, he is permitted to walk to a place known to him to take a siddur from there [the ח"א].
(8) 'He should not turn back for it' — because it distracts him. The Taz writes that the same applies, that one who prays from a siddur should be careful to mark beforehand all the places he will need for that particular prayer, and not to search for them while he is praying, for through this he will interrupt himself and will not concentrate well. And so too the ברכ"י wrote, that many men of (pious) deed conduct themselves this way, when they wish to say an added, special prayer, such as Aneinu, or such as Vatodi'einu when a Yom Tov falls at the conclusion of Shabbos (motzaei Shabbos).
(9) 'To pray from it' — it is found in the later authorities (Acharonim) that even during the chazzan's repetition (chazaras hashatz), it is proper that the siddur be open before him, so that his ears are attentive to what the chazzan says.
