Mishnah Berurah · Orach Chaim

The Shulchan Aruch rules: one should align his feet together, one right next to the other, precisely, as though they were not two feet but only one, in order to resemble the ministering angels, of whom it is written (Yechezkel 1) 'and their feet were a straight foot' (a single upright leg), meaning that their feet appeared to be one single leg. (And some say that when one rises to pray he should first step three steps forward, as a way of drawing near and approaching the matter he is about to perform.) (This is from the Rokeach.)

One must bend his head slightly, so that his eyes are directed downward, toward the ground, and he should picture himself as though he were standing in the Beis HaMikdash, while in his heart he directs his thoughts upward, toward Heaven.

One places his hands upon his heart, bound together (that is, as if tied), the right hand over the left, and he stands like a servant before his master, in awe, in fear, and in dread. He should not rest his hands upon his hips, because that is an arrogant posture. (And see further on, at the beginning of siman 97.)

It is good to align one's feet together also at the time when he recites Kedushah together with the chazzan.

(1) 'He should align his feet, etc.' — the Mishnah Berurah notes that after the fact, even if he did not align his feet at all, he has still fulfilled his obligation.

(2) 'To resemble the angels' — since he is speaking with the Shechinah, he must clear all bodily thoughts from his heart and imagine himself as though he were an angel. Even if he is seated in a wagon, he should nonetheless align his feet (so writes the Pri Megadim). They further wrote that he should not lean backward at that time, nor be tilted to the sides, and he should not stretch out his legs, nor cross one over the other, because all of this is a manner of haughtiness; rather he should sit with his head bent (see the Taz).

(3) 'He should step forward' — he need not first step backward in order then to step forward; so wrote the Eliyah Rabbah, but the common custom is indeed to step backward first. It is written in the Darchei Moshe that the Maharil would take his stance for the Shemoneh Esrei at Shacharis at the moment the chazzan began 'Tehilos la'Kel Elyon,' at Minchah when the chazzan went down before the ark, and at Maariv when the chazzan began the Kaddish. He should also, at that point, clear away his phlegm and mucus and anything that would disturb his concentration (so write the Acharonim).

(4) 'That his eyes be, etc.' — see above in siman 90, se'if katan 8, in the Mishnah Berurah; and the Bach wrote that before he begins to pray he should gaze toward the windows, upward toward Heaven, so that his heart will be humbled.

(5) 'Downward toward the ground' — and those who raise their heads and eyes upward, as though gazing at the ceiling — the angels mock them (so brought in ס"ח, siman 18). The Pri Megadim wrote that one should not make any strange movements; when praying alone it is permitted, but not in a congregation, and especially one should not raise his voice and the like. The Acharonim wrote that whoever does not close his eyes during the Shemoneh Esrei does not merit to behold the countenance of the Shechinah when his soul departs; but if he prays from a siddur with his eyes open in order to read from it, there is no objection to that. And see above in siman 91, se'if katan 6, in the Mishnah Berurah.

(6) 'His heart' — this applies in a place where it is customary to stand this way before the king when speaking with him and requesting one's needs from him; and it all follows the custom of the locale. In the sefer Asarah Maamaros it is written in the name of the Arizal that one should place his arms upon his heart, the right over the left (see there), and in the name of the Ramak, that he should tuck the thumb inside the palm of the hand.

(7) 'In awe' — one should sway during prayer, on account of the verse 'all my bones shall say, Hashem, who is like You,' (so writes פר"ח in the name of several poskim, and so too the Magen Giborim concurred).

(8) 'Also at the time, etc.' — for we recite 'just as they sanctify Him in the heavens above, etc.'
