כשיצא מבית הכסא יברך אשר יצר את האדם בחכמה שבריאת האדם היא בחכמה נפלאה וי"מ על שם שהגוף דומה לנוד מלא רוח והוא מלא נקבים כדלקמן בסמוך וי"מ בחכמה שהתקין מזונותיו של אדם הראשון ואח"כ בראו וברא בו נקבים נקבים אחלולים חלולים פירוש נקבים רבים כגון פה וחוטם ופי טבעת וגם ברא בו אברים רבים חלולים כמו לב וכרס ומעים. בשאם יסתם אחד מהם גכלומר שבנקבים יש נקב א' שהוא הפה כשהוא במעי אמו הוא סתום וכשיוצא לאויר העולם הוא נפתח ואם כשיוצא לאויר העולם היה נשאר סתום לא היה אפשר להתקיים אפי' שעה אחת והאברים החלולים אם היה נפתח אחד מהם לא היה אפשר להתקיים אפי' שעה אחת. ועוד יש לפרש שגבול יש לאדם שיכולין נקביו ליסתם ולא ימות וכיון שעבר אותו הגבול דאי אפשר להתקיים אפילו שעה אחת וכיון שבכלל הנקבים הם פי הטבעת ופי האמה ובכלל האברים החלולים שאם יפתח אחד מהם אי אפשר להתקיים הם כרס ומעים שפיר הוי שבח זה מענין עשיית צרכיו ואפשר עוד שמאחר שאם יוצא לנקביו ביותר עד שאם עבר הגבול ימות בכלל שאם יפתח אחד מהם הוא. והוי שאם יפתח אחד מהם נמי מענין עשיית צרכיו ממש. הרופא חולי כל בשר ע"ש הנקבים שברא בו והוציא פסולת מאכלו כי אם יתעפש בבטן ימות והוצאתו הוא רפואה. ומפליא לעשות מפני שהאדם דומה לנוד מלא רוח ואם יעשה אדם בנוד נקב כחודה של מחט הרוח יוצא והאדם מלא נקבים ורוחו משתמרת בתוכו הרי זה פלא ועוד יש לפרש על שם שבורר טוב המאכל ודוחה הפסולת: הגה ועוד יש לפרש שמפליא לעשות במה ששומר רוח האדם בקרבו וקושר דובר רוחני בדבר גשמי והכל הוא ע"י שהוא רופא כל בשר כי אז האדם בקו הבריאות ונשמתו משתמרת בקרבו (דברי עצמו):
When one leaves the bathroom, he recites the blessing 'asher yatzar' — 'who formed man with wisdom.' One explanation of 'with wisdom' is that the creation of the human being was done with wondrous wisdom. And some explain it refers to the fact that the body is comparable to a skin-flask full of air, yet it is full of openings — as will be explained just below. And some explain 'with wisdom' means that He prepared the food supply of Adam, the first man, and only afterward created him. The blessing continues: 'and created within him many openings and many hollow cavities.' That is: many openings, such as the mouth, the nose, and the anus. And He also created within him many hollow organs, such as the heart, the stomach, and the intestines. The blessing continues: 'that if one of them were to be blocked' — meaning: among the openings there is one opening, namely the mouth, which, while the fetus is in its mother's womb, is closed, and when he emerges into the air of the world it opens. And if, upon emerging into the air of the world, it were to remain closed, it would be impossible to survive even a single hour. And as for the hollow organs, if one of them were to become open, it would be impossible to survive even a single hour. And a further explanation: there is a limit for a person, within which his openings can be blocked and he will not die, but once that limit has been passed, it is impossible to survive even a single hour. And since included among 'the openings' are the anus and the urinary opening, and included among 'the hollow organs, that if one of them opens it is impossible to survive' are the stomach and the intestines, then this praise fits well with the subject of relieving oneself. And it is further possible that, since a person who relieves himself excessively — to the point that if he passed the limit he would die — is included in 'that if one of them opens.' So 'that if one of them opens' is likewise directly connected to the matter of relieving oneself. The blessing continues: 'who heals all flesh' — so called on account of the openings He created in him, through which the waste of his food is expelled, for if it were to putrefy in the belly he would die, and its expulsion is a healing. The blessing continues: 'and does wonders' — because a person is comparable to a skin-flask full of air, and if a person makes in such a flask a hole even the size of a needle's point, the air escapes; yet a person is full of openings, and his air is nonetheless retained within him — this is indeed a wonder. And a further explanation: it is on account of the fact that the body sorts out the good part of the food and expels the waste. The Rema adds: A further explanation — that 'does wonders' refers to the way He preserves the spirit of a person within him, binding a spiritual thing (the soul) to a physical thing (the body). And all of this comes about through His being 'the healer of all flesh,' for then the person is in a state of health and his soul is preserved within him. (These are the Rema's own words.)

There are those who have the practice of waiting to recite the blessing 'al netilas yadayim' (over washing the hands) until they arrive at the synagogue, and they arrange it together with the rest of the blessings. But the Sephardim did not follow this practice. The Rema adds: In any case, one should not recite the blessing twice; so one who recites it at home should not recite it again in the synagogue, and likewise one who recites it in the synagogue should not recite it at home (Kol Bo, siman 2). And one who studies before entering the synagogue, or who prays beforehand, should recite it at home and not in the synagogue. And even in such a case, there are those who have the practice of arranging it together with the rest of the blessings in the synagogue and do not recite it at home (Maharil, Laws of Prayer).
ברכת אלהי נשמה אינה פותחת בברוך מפני שהיא יבברכת ההודאה וברכת ההודאות אין פותחת בברוך כמו שמצינו בברכת הגשמים:
The blessing of 'Elokai Neshama' does not open with the word 'Baruch' (Blessed), because it is a blessing of thanksgiving, and a blessing of thanksgiving does not open with 'Baruch,' as we find in the blessing recited over rain.
יגיש נוהגין שאחר שבירך א' ברכת השחר וענו אחריו אמן חוזר א' מהעונים אמן ומברך ועונין אחריו אמן וכסדר הזה עושין כל אותם שענו אמן תחלה ואין לערער עליהם ולומר ידשכבר יצאו באמן שענו תחלה מפני שהמברך אינו מכוין להוציא אחרים ואפילו אם היה המברך מכוין להוציא אחרים הם מכונים שלא לצאת בברכתו:
There are those who have the practice that after one person has recited the morning blessings and the others have answered 'Amen' after him, one of those who answered 'Amen' then goes back and recites the blessings himself, and the others answer 'Amen' after him, and in this same order all those who answered 'Amen' the first time each do the same. And one should not object to them and say that they have already fulfilled their obligation with the 'Amen' they answered the first time, because the one reciting the blessing does not intend to discharge the obligation of others, and even if the one reciting were intending to discharge others' obligation, they intend not to fulfill their obligation through his blessing.
משנה ברורה
(א) חלולים - פי' איברים חלולים ולא יאמר חללים דבחלל לא שייך בריאה:
Seif katan 1, on the words 'chalulim' (hollow): This means hollow organs — one reads the word as 'chalulim' (hollow ones). One should not read it as 'chalalim' (empty spaces/voids), for regarding an empty void the term 'creation' is not applicable.
(ב) שאם יסתם וכו' - כן היא גי' הרמב"ם אבל גירסא שלנו ורש"י ורי"ף והרא"ש אם יפתח קודם [הגר"א]:
Seif katan 2, on 'that if one of them were blocked, etc.': This is the version of the Rambam, but our version, and that of Rashi, the Rif, and the Rosh, reads 'that if one of them were to open' — as noted earlier [the Gra].
(ג) כלומר שבנקבים וכו' - משום דגירסתו היה א"א להתקיים אפילו שעה אחת וקשה לו דאדם סותם פיו הרבה שעות ולזה קאמר כלומר וכו' אבל מנהגינו שלא לומר אפילו שעה אחת כמש"כ הר"מ וכן מוכח באגודה כ"כ המ"א וכן משמע בביאור הגר"א ועיין ט"ז:
Seif katan 3, on 'meaning that among the openings, etc.': The Mechaber added this explanation because his version of the blessing read 'it would be impossible to survive even a single hour,' and this posed a difficulty for him, since a person keeps his mouth closed for many hours at a time, and it was to resolve this that he said 'meaning, etc.' But our custom is not to say the words 'even a single hour,' as the Rambam wrote, and so is proven from the Agudah — so wrote the Magen Avraham — and so is implied in the commentary of the Gra; and see the Taz.
(ד) אי אפשר - אי בצירי תחת האלף כי המלה נגזרת מן אין כ"כ הבה"ט והפמ"ג ובשע"ת מיישב הגירסא בחירק שג"כ כונתו כמו אין כמו דכתיב אי כבוד שפירש"י שם שכונתו כמו אין כבוד:
Seif katan 4, on the words 'i efshar' (it is impossible): The word 'i' is pronounced with a tzeirei vowel under the aleph, since the word is derived from 'ein' (there is not). So wrote the Be'er Heitev and the Peri Megadim. And in the Sha'arei Teshuva he settles the reading with a chirik vowel, which likewise has the sense of 'ein' (there is not), just as it is written 'i chavod,' where Rashi explains there that its meaning is 'ein kavod' — 'there is no honor.'
(ה) רופא (חולי) - ב"ח ושל"ה ומט"מ כתבו שאין לומר חולי רק רופא כל בשר וט"ז כתב דשני הגירסאות הן נכונות וחלילה למחוק אחת מהן עיי"ש:
Seif katan 5, on '(who heals) [the sickness of] all flesh': The Bach, the Shelah, and the Mateh Moshe wrote that one should not say the word 'choli' (sickness), but only 'rofei kol basar' (who heals all flesh). But the Taz wrote that both versions are correct, and it would be wrong to erase either one of them — see there.
(ו) דבר רוחני - ובכוונת כתוב דהנשמה נהנית מרוחניות המאכל והגוף נהנה מגשמיות המאכל ומכח זה קשורים זה בזה ע"י המאכל [מ"א]:
Seif katan 6, on 'a spiritual thing': And in the Kavanos it is written that the soul derives benefit from the spiritual dimension of the food, while the body derives benefit from the physical dimension of the food, and by virtue of this they are bound to one another through the food [Magen Avraham].
(ז) להמתין - קבלה מר"י חסיד היוצא מביתו לבהכ"נ קודם שיתפלל יאמר פסוק שמע ישראל אבל יכוין שלא לצאת בזה המ"ע דק"ש אא"כ ירא שיעבור זמן ק"ש ועיין לקמן בסוף סימן מ"ו מה שכתבנו במ"ב שם:
Seif katan 7, on 'to wait': There is a tradition from Rabbi Yehuda HeChasid: one who leaves his home for the synagogue before he has prayed should recite the verse 'Shema Yisrael,' but he should have in mind not to fulfill the positive commandment of reciting the Shema with this — unless he is afraid that the proper time for reciting the Shema will pass. And see later, at the end of siman 46, what we wrote in the Mishnah Berurah there.
(ח) לברך - ודוקא אם הולך מיד לבהכ"נ אבל אם לומד אחר הנטילה וכ"ש כשמפסיק בשיחה בטלה ואח"ז הולך לבהכ"נ ומברך שם ענט"י לכו"ע לא עביד שפיר דיש הפסק גדול בין נטילה לברכה וכדלקמיה ע"כ אותם שאומרים תהלים קודם ענט"י לא יפה הם עושים דהוי הפסק גדול. כתב הלבוש אותם שאומרים תהלים טוב יותר שיאמרו קודם התפלה ע"ש הטעם:
Seif katan 8, on 'to recite the blessing': This practice of waiting applies specifically when he goes immediately to the synagogue, but if he studies Torah after the washing — and all the more so if he interrupts with idle conversation — and afterward goes to the synagogue and recites 'al netilas yadayim' there, then according to all opinions he has not acted properly, since there is a large interruption between the washing and the blessing, as will be explained below. Therefore, those who recite Tehillim before the blessing on washing the hands are not acting well, for it constitutes a large interruption. The Levush wrote that those who recite Tehillim would do better to say it before the Amida prayer — see there for the reason.
(ט) לא נהגו כן - והסכמת האחרונים לנהוג כאנשי ספרד שלא להפסיק לכתחלה בין ברכת ענט"י והנטילה אלא מיד שנוטל ידיו צריך לברך ענט"י אך אם בעת שנוטל ידיו הוא צריך לנקביו שאפילו בד"ת אסור ואפילו יכול להעמיד עצמו עד פרסה אין לו לברך ענט"י עד שיעשה צרכיו ויברך ענט"י ואשר יצר [שערי תשובה] ועיין לקמן בסי' צ"ב ס"א בהג"ה ובמ"ב שם ועיין לעיל בסימן ד' במשנה ברורה שכתבנו שם בשם הח"א שלכתחלה יותר טוב להמתין מלברך ענט"י עד לאחר שמנקה עצמו ורוצה להתפלל דאז יוצא לכו"ע ועיין שם בביאור הלכה שכתבנו דטוב שאז יסדר אחר ברכת ענט"י ואשר יצר גם שאר ברכות השחר ובאופן זה מיירי הח"א שכתב דיוצא לכו"ע. ועיין שם בביאור הלכה שבמעשה רב כתב ג"כ כהח"א ומ"מ העושה כהשע"ת יש לו ג"כ על מה לסמוך:
Seif katan 9, on 'did not follow this practice': The consensus of the later authorities is to follow the practice of the Sephardim, not to make an interruption, at the outset, between the blessing on washing the hands and the washing itself, but rather, immediately upon washing his hands, he must recite 'al netilas yadayim.' However, if at the time he washes his hands he needs to relieve himself, in a case where it would even be forbidden to speak words of Torah, and even if he is able to hold himself back for as long as it takes to walk a parsah, he should not recite 'al netilas yadayim' until he relieves himself, and then he recites both 'al netilas yadayim' and 'asher yatzar' [Sha'arei Teshuva]. And see later in siman 92, se'if 1, in the Rema's gloss, and in the Mishnah Berurah there. And see earlier in siman 4, in the Mishnah Berurah, where we wrote in the name of the Chayei Adam that, at the outset, it is better to wait to recite 'al netilas yadayim' until after he has cleaned himself and is ready to pray, for then he fulfills his obligation according to all opinions. And see there in the Bi'ur Halacha, where we wrote that it is good, at that point, to arrange after the blessing of 'al netilas yadayim' and 'asher yatzar' also the rest of the morning blessings, and it is in such a manner that the Chayei Adam is speaking when he wrote that one fulfills his obligation according to all opinions. And see there in the Bi'ur Halacha that in the Ma'aseh Rav it is likewise written in accordance with the Chayei Adam, but nonetheless, one who acts in accordance with the Sha'arei Teshuva also has authority on which to rely.
(י) שמברכם בביתו - עיין במ"א ופמ"ג ושארי אחרונים שהסכימו דאפילו ש"ץ שבירך בביתו שוב לא יברך בבהכ"נ דבימינו הכל בקיאים וכל אחד מברך לעצמו ואין רוצה לצאת בברכת הש"ץ ע"כ במקום שהמנהג שהש"ץ אומר כל הברכות בצבור כמו במקומותינו שהמנהג כן בימים נוראים שמברך בצבור גם ענט"י ואשר יצר וברכת התורה יזהר מתחלה שלא יברך אותם בביתו. ואם אירע שכבר בירך אותם בביתו יראה לבקש לאחד מהשומעים שעדיין לא בירך שיכוין לצאת בברכתו אך אז יהיה זהיר הש"ץ מאוד להשמיע כל לשון הברכה ולא כמו שנוהגין באיזה מקום שהש"ץ משמיע רק לשון תחלת הברכה וסופה דכיון שכונתו אז בהברכה להוציא צריך השומע להשמיע כל לשון הברכה:
Seif katan 10, on 'one who recites them at home': See the Magen Avraham, the Peri Megadim, and other later authorities, who agreed that even a chazzan (prayer leader) who recited the blessings at home should not recite them again in the synagogue, since nowadays everyone is proficient and each person recites the blessings for himself and does not wish to fulfill his obligation through the chazzan's blessing. Therefore, in a place where the custom is that the chazzan recites all the blessings aloud with the congregation — as in our locales, where such is the custom on the High Holy Days, that he recites aloud also 'al netilas yadayim,' 'asher yatzar,' and the blessings of the Torah — he should be careful from the outset not to recite them at home. And if it happened that he already recited them at home, he should see to it to ask one of the listeners who has not yet recited the blessings to have in mind to fulfill his obligation through the chazzan's blessing, but then the chazzan must be very careful to make audible the entire wording of the blessing, and not as they practice in some places, where the chazzan makes audible only the wording of the beginning and the end of the blessing, for since his intention with the blessing at that point is to discharge the listener's obligation, the listener must hear the entire wording of the blessing.
(יא) בכה"ג וכו' - היינו מה שנוהגין רוב המון העם להתפלל סליחות קודם אור הבוקר בימי הסליחות ושם מזכירין כמה פסוקים ואח"כ מברכין ענט"י וברכת התורה ולענין ברכת התורה יש ליישב קצת דאותן הפסוקים אין אומרים אותן אלא דרך תפלה ולא נתחייבו בהם בבה"ת לפי דעת איזו פוסקים כמו שנתבאר בסימן מ"ו ס"ט בהג"ה ועיין שם במ"ב אך לענין נט"י שמפסיקין בהפסקה כזאת צ"ע ליישב והסכימו האחרונים שלא לעשות כן אלא יש לברך בביתו מיד בקומו ושוב אין לברך ענט"י בבהכ"נ וכמו שנתבאר בסימן זה ס"א או שבעת קומו לא יברך ענט"י אלא אחר גמר הסליחות יבדוק עצמו לנקביו או יטיל מים וישפשף ואז יברך ענט"י ואשר יצר ויתפלל וכמו שכתבנו לעיל בשם הח"א:
Seif katan 11, on 'even in such a case, etc.': This refers to what the majority of the common people practice — to pray Selichos before the light of morning during the days of Selichos, and there they recite a number of verses, and afterward they recite 'al netilas yadayim' and the blessings of the Torah. As for the blessings of the Torah, the matter can be somewhat settled, since those verses are recited only in the manner of prayer, and one is not obligated to recite the blessings of the Torah for them, according to the opinion of some authorities, as explained in siman 46, se'if 9, in the Rema's gloss — and see there in the Mishnah Berurah. But with regard to the washing of the hands, since they make such an interruption, it requires further study to justify, and the later authorities agreed not to do so. Rather, one should recite the blessing at home immediately upon rising, and then he should not recite 'al netilas yadayim' again in the synagogue, as explained in this siman, se'if 1; or, at the time of rising he should not recite 'al netilas yadayim,' but rather after finishing the Selichos he should check himself for his bodily needs, or urinate and rub his hands, and then recite 'al netilas yadayim' and 'asher yatzar' and pray — as we wrote earlier in the name of the Chayei Adam.
(יב) ברכת ההודאה - ואינה ברכה הסמוכה לחברתה ולכן רשאי לומר בבוקר בקומו עד אלהי נשמה ובבהכ"נ יתחיל אלהי נשמה [טור] ומ"מ יש לעשות על צד היותר טוב להסמיך אלהי נשמה לברכת אשר יצר ועיין בביאור הגר"א בסימן מ"ו שדעתו ג"כ דברכת אלקי נשמה סמוכה לברכת אשר יצר ע"כ נכון ליזהר בזה לכתחלה. אם לא בירך ברכת אלהי נשמה עד אחר שהתפלל י"ח אינו מברך עוד לאחר התפלה שכיון שכבר אמר מחיה המתים הוא דומה למחזיר נשמות [פר"ח] ובשע"ת מפקפק מאוד בדין זה ומסיק דעכ"פ אם בעת שאמר ברכת מחיה מתים היה בדעתו לברך אלקי נשמה אחר התפלה רשאי אח"כ לברך עי"ש הטעם וכן משמע בפמ"ג סי' נ"ב ועיין מה שכ' שם בביאור הלכה. בנוסח הברכה לא יאמר אתה יצרת בי רק אתה יצרת אתה נפחת בי:
Seif katan 12, on 'a blessing of thanksgiving': And it is not a blessing that is 'adjacent to its fellow' (i.e. part of a chain of blessings), and therefore one is permitted to recite, in the morning upon rising, up to 'Elokai Neshama,' and in the synagogue to begin with 'Elokai Neshama' [Tur]. Nonetheless, it is best to act in the preferable manner and place 'Elokai Neshama' immediately after the blessing of 'asher yatzar,' and see in the commentary of the Gra in siman 46 that his opinion, too, is that the blessing of 'Elokai Neshama' is adjacent to the blessing of 'asher yatzar,' and therefore it is proper to be careful about this at the outset. If one did not recite the blessing of 'Elokai Neshama' until after he prayed the Shemoneh Esrei, he does not recite it any longer after the prayer, since, once he already said 'who revives the dead,' it resembles the theme of 'who returns souls' [Peri Chadash]. But in the Sha'arei Teshuva he expresses strong doubt about this ruling, and concludes that in any case, if at the time he said the blessing 'who revives the dead' he had in mind to recite 'Elokai Neshama' after the prayer, he is permitted to recite it afterward — see there for the reason, and so is implied in the Peri Megadim, siman 52; and see what is written there in the Bi'ur Halacha. In the wording of the blessing, one should not say 'You formed it within me,' but rather 'You formed it, You breathed it into me.'
(יג) יש נוהגין וכו' - וכונתם הוא שישלים כ"א עד צ' אמנים ליום ועיין בש"ת שיש שמפקפקין בזה ובתשובה מאהבה [ח"ב סי' רי"ב] העלה דלדידן שנוהגין שלא לצאת בברכת הש"ץ לכו"ע מותר לעשות כן ובפמ"ג כתב דראוי עתה שלא לומר בקול רם כ"א הש"ץ עי"ש ע"כ כל מקום ומקום יעשה כפי מנהגו שכ"א יש לו על מי לסמוך:
Seif katan 13, on 'there are those who have the practice, etc.': Their intention is that each person should complete a total of ninety 'Amen' responses per day. And see in the Sha'arei Teshuva that there are some who cast doubt on this. And in the Teshuva MeAhava [part 2, siman 212] he concluded that for us, who have the practice not to fulfill our obligation through the chazzan's blessing, it is permitted according to all opinions to do so. And in the Peri Megadim it is written that it is fitting nowadays that only the chazzan should recite them aloud — see there. Therefore, each and every place should act in accordance with its own custom, for each has authority on which to rely.
(יד) שכבר יצאו - בלבוש משמע שאין יכול להוציא ברכת השחר בפחות מעשרה ועיין בפמ"ג דמצדד בסוף דבריו לומר דדוקא בבקי אין מוציא בפחות מעשרה אבל בשאין בקי א"צ עשרה אף להלבוש:
Seif katan 14, on 'that they have already fulfilled their obligation': In the Levush it is implied that one cannot discharge the obligation of the morning blessings for others with fewer than ten men, and see in the Peri Megadim, who inclines at the end of his words to say that specifically for one who is proficient can one not discharge the obligation with fewer than ten, but for one who is not proficient, ten are not required even according to the Levush.
