היתה טלית חגורה על מתניו לכסותו ממתניו ולמטה אסור להתפלל אעד בשיכסה לבו ואם לא כסה את לבו או שנאנס ואין לו במה יתכסה גהואיל וכסה ערותו והתפלל יצא:
If a garment (talit) was girded around his loins, covering him from the loins downward, it is nevertheless forbidden to pray until he also covers his heart (chest); but if he did not cover his heart, or he was prevented by circumstances and has nothing with which to cover himself, then since he did cover his nakedness and prayed, he has fulfilled his obligation after the fact.
ס״ק אעד וכו'
(א) עד וכו' - דצריך הוא לראות את עצמו כאלו עומד לפני המלך ומדבר באימה:
(1) 'Until [he covers his heart]' — for one must picture himself as though he is standing before the King, and speaking with awe.

(ב) שיכסה לבו - והוא הדין כל גופו [ב"ח]:
(2) 'That he cover his heart' — and the same law applies to his whole body, which should likewise be covered (from the Bach).

(ג) הואיל וכו' - ר"ל אפי' הי' ממתניו ולמעלה ערום יצא:
(3) 'Since, etc.' — that is to say, even if he was naked from the loins upward, he has fulfilled his obligation after the fact.
דצריך לאזור אזור בשעת התפלה אפי' יש לו האבנט שאין לבו רואה את הערו' ומשום הכון אבל זשאר ברכות מותר לברך בלא חגורה מאחר שיש לו חמכנסים: הגה ואין לבו רואה את הערוה [ר"ן פרק קמא דשבת והרא"ש פרק הרוא' והגהת מיימוני פרק ה' מהלכות תפלה]:
One must gird himself with a belt (ezor) at the time of prayer, even if he already has a sash (avnet) so that his heart does not 'see' his nakedness, because of the verse 'Prepare to meet your God' (which calls for readiness before Him). But for other blessings it is permitted to bless without a belt, since he has trousers (which already separate his heart from his nakedness). The Rema adds: [the belt is required] so that his heart not 'see' his nakedness (from the Ran, first chapter of Shabbos, the Rosh in chapter 'HaRo'eh,' and Hagahos Maimoni, chapter 5 of Hilchos Tefillah).

(ד) צריך - בדיעבד אם התפלל בלא אזור יצא וי"א עוד דדוקא מי שרגיל כל היום בחגורה אבל מי שהולך כל היום בלא חגורה גם בשעת תפלה א"צ לחגור. ומיהו מדת חסידות אף בכה"ג [מג"ג]:
(4) 'One must' — after the fact, if he prayed without a belt, he has fulfilled his obligation. And some further say that this applies specifically to one who is accustomed to a belt the whole day, but one who goes about all day without a belt need not gird himself even at the time of prayer. Nevertheless, it is a measure of piety [to gird oneself] even in such a case (from מג"ג).

(ה) אבנט - ר"ל אבנט של מכנסים המפסיק בין לבו לערוה:
(5) 'A sash (avnet)' — that is to say, the sash of his trousers, which forms a separation between his heart and his nakedness.

(ו) משום הכון לקראת וגו' - איתא בזוהר ואתחנן מאן דקאים בצלותא בעי לכסויי רישיה ועינוי בגין דלא יסתכל בשכינתא. [והיינו בטלית של מצוה וכן נוהגין כהיום לכסות ראש עד עינים בטלית מצוה בשעת תפלת י"ח. פמ"ג] מאן דפקח עינוי בשעת צלותיה מקדים עליה מה"מ וכו' ועיין בסי' צ"ה במ"ב סק"ה:
(6) 'Because of "Prepare to meet," etc.' — it is stated in the Zohar (parashas Va'eschanan): one who stands in prayer must cover his head and his eyes, so that he not gaze at the Divine Presence. [And this refers to the talit of the mitzvah; and so the custom nowadays is to cover the head down to the eyes with the mitzvah-talit during the Shemoneh Esrei. The Pri Megadim.] One who opens his eyes during his prayer brings the Angel of Death upon himself prematurely, etc. And see in Siman 95, in the Mishnah Berurah, se'if katan 5.

(7) 'Other blessings' — and likewise the Shema, for 'Prepare to meet your God' applies only when one stands before the King (in the Amidah).

(ח) מכנסים - וה"ה אם בגדיו מונחים דבוקים ממש על הבטן ומפסיקים בין לבו לערוה:
(8) 'Trousers' — and likewise if his garments lie pressed right against his belly and thereby separate between his heart and his nakedness.
יש אומרים שאסור להוציא הזכרה מפיו בראש מגולה וי"א שיש למחות שלא ליכנס בב"ה טבגלוי הראש:
Some say that it is forbidden to utter a mention of God's Name from one's mouth while bareheaded, and some say that one should protest against anyone entering the synagogue (beis haknesses) with an uncovered head.

(ט) בגילוי הראש - ועיין בבה"ל:
(9) 'With an uncovered head' — and see the Biur Halacha.
כובעים [קפיל"ה בלעז] הקלועים מקש חשיבא כסוי אבל הנחת יד על הראש ילא חשיבא כסוי ואם אחר מניח ידו על ראשו של זה משמע דחשיבא כסוי:
Hats (called 'capila' in the vernacular) that are woven from straw are considered a valid covering [for the head], but merely placing one's hand upon the head is not considered a covering; yet if another person places his hand upon this one's head, it would seem that this is considered a covering.
ס״ק ילא חשיב וכו'
(י) לא חשיב וכו' - דהראש והיד חד גוף אינון ואין הגוף יכול לכסות עצמו. וכתב הב"ח מיהו נוהגין להמשיך הבית יד של הבגד על היד ומכסה בו ראשו ושפיר חשיב כיסוי:
(10) 'Is not considered, etc.' — for the head and the hand are one body, and the body cannot cover itself. But the Bach wrote that nevertheless the custom is to draw the sleeve of the garment over the hand and cover the head with it, and this is properly considered a covering.
לא יעמוד יאבאפונדתו [טאסקא בלעז] ולא בראש יבמגולה ולא ברגלים יגמגולים אם דרך אנשי המקום שלא יעמדו לפני הגדולים אלא בבתי רגלים:
One should not stand [in prayer] wearing his money-belt (apunda, called 'taska' in the vernacular), nor with an uncovered head, nor with bare feet — this last, if the custom of the local people is not to stand before great men except with foot-coverings.

(יא) באפונדתו - הוא איזור חלול שמשימין בו מעות ואיננו דרך כבוד לעמוד כן לפני הש"י. וכן אין נכון להתפלל בקאפטין ובגד התחתון או בשלאף ראק. [ח"א]:
(11) 'With his money-belt (apunda)' — this is a hollow belt in which one places coins, and it is not a respectful way to stand thus before the Holy One, blessed be He. And likewise it is not proper to pray in a kaftan or under-garment, or in a dressing gown (Schlafrock) (from ח"א).

(יב) מגולה - ובזמנינו צריך להשים בעת התפלה כובע בראשו כדרך שהולך ברחוב ולא בכובע הקטן שתחת הכובע כי אין דרך לעמוד כן לפני אנשים חשובים וכ"ש שלאף מיץ. אין להתפלל בבתי שוקיים של פשתן לבד דגנאי הוא לעמוד כן לפני גדולים ומכ"ש כשלובשין סנדל והעקב מגולה. גם אין ללבוש בעת התפלה בתי ידים שקורין הענטשי"ך כדרך עוברי דרכים וקורא אני עליהם אל תבואני רגל גאוה ויד רשעים אל תנידני [ב"ח] ומהח"א דלעיל משמע דאפילו בבתי שוקיים של צמר לבד ג"כ אין נכון והכל תלוי לפי מנהג המקומות. הרמ"מ תיקן שלא לכנס לבהכ"נ במנעלים ארוכים והיינו שטיוויל ואם דרך אותו המקום לעמוד כך בפני גדולים מותר [פמ"ג]:
(12) 'Uncovered' — and in our times one must place a hat on his head during prayer, in the manner in which he goes out in the street, and not [merely] with the small cap worn beneath the hat, for it is not the way to stand thus before important people, and all the more so [not] in a nightcap (Schlafmütze). One should not pray in linen leggings alone, for it is disgraceful to stand thus before great men, and all the more so when one wears a sandal with the heel exposed. Also, one should not wear during prayer gloves — called 'Handschuh' — in the manner of travelers on the road, and I apply to them the verse: 'Let not the foot of pride overtake me, nor the hand of the wicked drive me away' (from the Bach). And from the ח"א cited above it is implied that even in woolen leggings alone it is likewise improper — and it all depends on the custom of the various places. הרמ"מ instituted not to enter the synagogue in long shoes — that is, boots (Stiefel) — and if it is the custom of that place to stand thus before great men, it is permitted (the Pri Megadim).
ס״ק יגמגולים
(יג) מגולים - מיהו אם הבגד ארוך שחופה את הרגלים או בארצות החמין מאוד שעומדי' שם ג"כ בפני גדולים יחף אין לחוש אפילו הבגדים קצרים ונראין הרגלים:
(13) 'Bare [feet]' — however, if the garment is long, such that it covers the feet, or in very hot lands, where people stand there before great men barefoot as well, there is no concern, even if the garments are short and the feet are visible.
דרך החכמים ותלמידיהם שלא יתפללו אלא כשהם עטופים: הגה ובעת הזעם יש לחבק הידים בשעת התפלה כעבדא קמי מאריה ובעת שלום יש להתקשט בבגדים נאים להתפלל [טור]:
It is the way of the Sages and their disciples not to pray unless they are wrapped [in a cloak]. The Rema adds: In a time of [Divine] wrath one should fold his hands during prayer, like a servant before his master, and in a time of peace one should adorn himself in fine garments to pray (from the Tur).
