One who is certain of himself that he cannot control himself from breaking wind until he finishes the Shema and the Amidah it is better that the time of Krias Shema and the Amidah pass and he not pray than that he pray without a clean body and if the time for prayer passed, he is considered one prevented by circumstances and he prays Mincha twice and if it seems to him that he can control himself during the Shema he puts on tefillin between [the blessing of] Ahava and the Shema The Rema adds: and he recites a blessing over them [Tur]
ס״ק אשברי לו
(א) שברי לו - לאפוקי אם אינו ברי לו והוא משהא עצמו מחששא דשמא יבוא להפיח באמצע שלא כדין הוא עושה:
(1) 'That he is certain' — this comes to exclude the case where he is not certain [that he cannot control himself] and yet he holds himself back out of a worry that perhaps he will come to break wind in the middle — [in such a case] he is acting improperly

(2) 'That he cannot,' etc. — that is to say, even if he will recite only the Shema and the Amidah by themselves he still will not be able to restrain himself from breaking wind in the middle for otherwise it would be better to do this than to forgo entirely the Shemoneh Esrei prayer

(ג) זמן ק"ש - האחרונים הסכימו דאין לו לעבור זמן ק"ש שהוא דאורייתא משום זה וגם בתשובת הרא"ש שממנה מקור הדין לא נזכר שם ק"ש ובמגן גבורים כתב דהוא רק שיגרא דלישנא וראיה דבסימן ל"ח ס"ב איתא ג"כ זה הדין ולא נזכר שם רק תפלה לבדה וגם כאן סיים רק בתפלה לבדה וכונת השו"ע רק לומר דעי"ז ישנה הנהגתו שמתפלל תמיד ק"ש ותפלה בזמנה אלא יקרא ק"ש לבדה עם ברכותיה ולא יתפלל כי בתפלה הוא עומד לפני המלך וגנאי גדול הוא להפיח אז אבל ק"ש אינו כמדבר עם המלך אלא שמקיים בזה רצון הש"י ואף שיקרא ק"ש בלי תפילין שהתפילין צריכין גוף נקי לא ימתין ויעבור זמן ק"ש כדי לקרותה בתפילין אלא מתי שיוכל להניח תפילין ביום יניחם:
(3) 'The time of Shema' — the later authorities agreed that he should not let the time of Shema pass — which is a Torah obligation — on account of this and moreover, in the responsum of the Rosh, which is the source of this ruling, the Shema is not mentioned at all and in the Magen Giborim it is written that [the mention of Shema] is merely a slip of the pen (a habitual turn of phrase) and the proof is that in Siman 38, se'if 2, this same ruling is stated and there only the Amidah alone is mentioned, and here too [the author] concluded with the Amidah alone and the intent of the Shulchan Aruch is only to say that through this he changes his usual practice of always praying the Shema and the Amidah in their proper time rather, he reads the Shema by itself with its blessings and does not pray the Amidah because in the Amidah he stands before the King, and it is a great disgrace to break wind then but the Shema is not like speaking with the King; rather, through it he fulfills the will of Hashem and even though he will read the Shema without tefillin — since tefillin require a clean body he should not wait and let the time of Shema pass in order to read it with tefillin rather, whenever he is able to put on tefillin during the day, he puts them on

(ד) ומברך עליהן - ותיכף אחר ק"ש יחלצן אבל הטלית יכול ללבשו ולברך עליו קודם שיתחיל הברכות וגם יוכל ללבשו כל היום שא"צ גוף נקי לציצית ומיהו אם לא הניח הטלית מקודם ובא לו בין אהבה לק"ש יכול להניח בלא ברכה דלבישה לא הוי הפסק וכ"ז כשאירע לו מקרה זה בשביל שום מאכל שהזיק לו וכיוצא באופן שלערב בודאי יכול להתפלל בגוף נקי אבל מי שרגיל בכך שיש לו חולי שמחמתו מוכרח להפיח בכל שעה ואי לא יתפלל נפטר מן התפלה לעולם מותר לו להתפלל בלא גוף נקי דלא אתי איסור הפחה שהוא ריח רע שאין לו עיקר דהוא רק איסור דרבנן ודחי לק"ש דאורייתא או לתפלה שהוא מצוה דרבים לכן יקרא ק"ש ויתפלל [רמ"א בתשובה סי' צ"ח] והפר"ח כתב ג"כ ככה אלא דהוסיף דכשיגיע לו הפחה יפסיק עד שיכלה הריח כמו בריח רע שאין לו עיקר דפוסק ויחזור ויתפלל. והפמ"ג נסתפק עוד דאפשר דשרי לו בכה"ג להניח תפילין ג"כ דהאיך יעקור לעולם מ"ע דאורייתא דתפילין ע"ש ונ"ל דכונת הפמ"ג רק ההנחה על איזו רגעים ורק כדי לקיים המ"ע דיותר מזה בודאי אסור ואם אין יכול להעציר עצמו מלהפיח עד כדי שיניח השל יד והשל ראש יניחם אחד אחד. ולענין ברכה נ"ל דאל יברך עליהם. כתב רמ"א בתשובה סימן צ"ח מי שיש לו חולי שמטפטף תמיד מי רגלים לאונסו וא"כ לא יוכל להתפלל לעולם יש להתיר לעשות לו בגד סביב אברו והבגדים העליונים יהיו נקיים ויכנס לבהכ"נ ויתפלל ובשעת הטפת מי רגלים יפסיק ואח"כ יתפלל ודוקא שלא הוצרך לטפטף בשעה שמתחיל להתפלל ואע"פ שיודע שבתוך תפלתו יצטרך לטפטף וכו' ע"ש:
(4) 'And he recites a blessing over them' — and immediately after the Shema he removes them but the tallis — he may put it on and recite the blessing over it before he begins the blessings [of Shema] and he may also wear it the entire day, since tzitzis do not require a clean body however, if he did not put on the tallis beforehand and it came to him between [the blessing of] Ahava and the Shema he may put it on without a blessing, for [the act of] donning it is not considered an interruption and all this applies when this circumstance befell him because of some food that disagreed with him or the like — in a situation where by evening he will surely be able to pray with a clean body but one who is regularly in this state, who has an illness on account of which he is forced to break wind constantly, and who, if he does not pray [this way], would be exempt from prayer forever it is permitted for him to pray without a clean body for the prohibition of breaking wind — which is a bad odor that has no physical substance, being only a Rabbinic prohibition — does not come and override the Torah-level Shema or the prayer, which is a communal mitzvah therefore he reads the Shema and prays [Rema, in a responsum, siman 98] and the Pri Chadash wrote likewise, except that he added that when a breaking of wind comes upon him, he pauses until the odor dissipates just as with a bad odor that has no substance, where one pauses and then resumes and prays and the Pri Megadim was further in doubt, [suggesting] that perhaps it is permitted for him in such a case to put on tefillin as well — for how could he permanently uproot the Torah positive commandment of tefillin — see there and it seems to me that the Pri Megadim intends only putting them on for a few moments, and only in order to fulfill the positive commandment, for more than this is certainly forbidden and if he cannot hold himself back from breaking wind for as long as it takes to put on both the hand-tefillin and the head-tefillin, he puts them on one at a time and regarding a blessing, it seems to me that he should not recite a blessing over them The Rema wrote in a responsum, siman 98: One who has an illness that causes urine to dribble constantly against his will, so that he could never pray one should permit making for him a garment around his organ, with the outer garments kept clean, and he enters the synagogue and prays and at the time the urine dribbles he pauses, and afterward he prays and this is specifically when he did not need to dribble at the moment he begins to pray, even though he knows that during his prayer he will need to dribble, etc. — see there
