כל היום כשעושה צרכיו בין קטנים בין גדולים אמברך באשר יצר ולא על נטילת ידים אף אם רוצה ללמוד או גלהתפלל מיד: הגה היו ידיו מלוכלכות שדשפשף בהן א"ה אינו מברך על נטילת ידים: (סמ"ג סי' כ"ז מ"ט):

At any time of day, when a person relieves himself — whether urinating ("small needs") or moving his bowels ("large needs") — he recites the blessing of Asher Yatzar ("...who fashioned man"). but he does not recite the blessing of Al Netilat Yadayim ("...concerning the washing of hands"). But the Sha'arei Teshuvah, citing several Acharonim, agreed that if he only remembered after the urge to urinate again had already begun, he should no longer recite it; see there.

On the words "Asher Yatzar": The Maharshal wrote in a responsum (siman 97): One who ate something requiring an after-blessing and forgot, then urinated, and recalled the after-blessing just before he was about to recite Asher Yatzar — which of the two comes first? He answered that Asher Yatzar comes first, because it is the more frequent blessing (and the more frequent takes precedence). Regarding one who drinks a laxative, whose intention from the outset is to empty the intestines until all the waste matter is expelled: therefore, some say he should bless only after the entire process of elimination is complete (since he never diverted his mind in between), and some say he should bless each time; and the custom follows the latter opinion (to bless each time). However, in a case of strong diarrhea, where he feels he will immediately need to relieve himself again, it is forbidden to bless — as stated later in siman 92, that even divrei Torah are forbidden in such a state. On the words "to pray immediately": Even though we have established in siman 233, se'if 2, that for the Minchah prayer, and likewise for the evening prayer, one must wash his hands even when he is aware of no soiling on them — and all the more so when he has relieved himself — nevertheless he recites the blessing Al Netilat Yadayim only at the morning (Shacharit) washing. And see the sefer Artzos HaChaim, who wrote that in the view of most of the poskim, if one moved his bowels and afterward wishes to pray, he does recite Al Netilat Yadayim. Nonetheless, ideally one should not depart from the ruling of the Shulchan Aruch (that the blessing is said only in the morning). And see the sefer Maaseh Rav.

(ד) ששפשף בהן - הניצוצות של מי רגלים וה"ה בגדולים שעשה צרכיו וקנח:

On the words "that he rubbed with them": This refers to the droplets of urine (which soiled his hands when he rubbed). And the same applies in the case of a bowel movement, where he relieved himself and wiped.

סעיף ב׳

הטיל מים ולא שפשף אע"פ שצריך לברך אשר יצר אין צריך ליטול ידיו האלא משום נקיות או משום הכון:

If he urinated but did not rub with his hands at the place of relief — even though he is obligated to recite Asher Yatzar — he is not required to wash his hands for the purpose of a blessing, but only for the sake of cleanliness, or on account of "prepare yourself" (readying oneself to stand before God).

(ה) אלא משום וכו' - ואם אין לו במה ליטול אעפ"כ צריך לברך אשר יצר. ואפילו אם שפשף משמע לעיל בסוף סימן ד' במ"א סקכ"ח דמהני לברכות אם מנקה ידיו בכל מידי דמנקי:

On the words "but only for the sake, etc.": And if he has nothing with which to wash, even so he must recite Asher Yatzar (since the washing here is only for cleanliness, not a precondition for the blessing). And even if he rubbed with his hands — it is implied earlier, at the end of siman 4, in the Magen Avraham, se'if katan 28, that it suffices for blessings if he cleans his hands with any substance that cleans.

הטיל מים והסיח דעתו מלהטיל מים ואח"כ נמלך והטיל מים פעם אחרת צריך לברך וב"פ אשר יצר:

If he urinated and then turned his mind away from urinating again (concluding that he was finished), and afterward he reconsidered and urinated a second time, he must recite Asher Yatzar twice — once for each occasion.

There is no minimum quantity required for urination to obligate the blessing. For even for a single drop one is obligated to bless, because if the opening were blocked so that even that one drop could not come out, it would be difficult for him (a danger to his health); and therefore he is obligated to give thanks.
